Here is what would surprise most clinic owners. The name in that answer was almost never pulled from the clinic's website. Every generic optometry-marketing guide promises that a tidy site and some schema markup get a practice recommended by AI. That advice is not wrong so much as aimed at the wrong target. The engines are reading a layer above your website: directories, the Ontario Association of Optometrists register, review platforms, and Google's own local data. This study walks through the signals that actually decide the recommendation, in the order they matter, grounded in real Ontario sources and Matt Griffin's first-hand audits of optometry practices in this province, not borrowed American theory.
A note before we start: this is marketing research, not medical or optometric advice. Named platforms and registers are real; references to them describe how AI engines cite sources, not a clinical endorsement or a judgement on any practice's standard of care.
Why does AI name a directory before it names your clinic?
Because an AI engine builds a best-optometrist answer from third-party listing pages, professional registers and Google's local data, not from any single clinic's site. ChatGPT leans on Google. Its local shortlists read like a Google Maps pack with sentences attached, complete with addresses, hours and an "open now" cue pulled from Google's place data. For ChatGPT specifically, a complete Google Business Profile is the source you can actually see and act on. Independent reporting on how ChatGPT selects local businesses points the same way: it favours entities with strong, consistent Google presence and authoritative list mentions.
Gemini wraps its sources in Google's grounding pipeline. Gemini frequently attributes its answer to a Vertex AI Search grounding redirect rather than a named publisher, a pipe that carries many underlying sources at once. Behind the wrapper you find directories, the OAO register, review sites and the occasional clinic page, but the citation you see is the redirect, so structured, machine-readable pages are what let it ground you cleanly.
Perplexity spreads out. Perplexity tends to cite four to six sources in a single answer, mixing a top-rated directory list with a couple of practice sites and niche aggregators rather than committing to one layer. It is the engine where a well-structured clinic page has the best chance of appearing directly, alongside the directories.
Google's AI surface stays close to local data. For explicitly local health queries, Google's AI answers pull heavily from Business Profile and Maps data.
